Welcome!

It is the goal of the health office to promote student and staff wellness.  We have a team of staff that will provide care for illness or injury, administer medications, assess and communicate to parents, encourage self-care and protect students well being.

We have three dedicated health office professionals. You can reach them directly in the health office at 763-316-6257 or through [email protected].

  • Mrs. Talisha Heilman, RN, RNC-LRN, CLC & Mrs. Carrie Rademacher, RN, PHN staff the Health Office during regular school hours.
  • The Anoka-Hennepin District School Nurse will consult with our Health Office staff and families as applicable.

Are Your Kids Ready for School?

Students are required to receive certain vaccines for school or submit an exemption. This requirement applies for all public, private, online, and home schools in Minnesota. Look for your child’s grade in the chart and see how many total doses of each vaccine are needed for their grade.

If you conscientiously object to any of the required immunizations please indicate which ones on this form and have it notarized before submitting to the School Health Office.
